We have developed two forms to help providers give us the relevant information to cancel their care service registration. 

 Childminder Application to Cancel - for childminders only

 All Other Services Application to Cancel - for all other services

You can print and complete this form and return it to your local office, or you can telephone your local office and ask for a copy to be sent to you.
